Student-Athlete Participation Forms

• IMPORTANT NOTICE  Regarding Activity Registration for Fall Sports •

Our athletics website provider has launched a new system, so the start of fall sports registration will be POSTPONED UNTIL THE FIRST WEEK OF JUNE. This will allow us time to test the new system and edit the instructions for a better experience. We will notify you when the Activity Registration is fully functional. automatic notifications. This registration portal will allow you to create a profile for your student-athlete, complete and upload
required forms and easily update any information for your student-athlete throughout the school year.

**  The Student-Athlete Participation Forms must be completed and signed for each athlete each school year prior to tryouts
in order to participate in the Athletics program at Carmel Christian School.  **

Athletics Physical:
2025-2026 Athletic Physical Form: (This form must be on file before tryouts each year.  It requires a physician’s signature.  CLICK HERE to download the physical form, print and take with you to your physician. Upload your completed physical form through the Athletic Participation button above. 

ImPACT Testing:
Who: Rising freshmen and new CCS High School student-athletes
Overview: ImPACT testing is required for participation in any sport at CCS.
When: ImPACT testing must be completed by before the first game.
Why: It is imperative to treat every concussion individually and to implement baseline testing. ImPACT will track information such as memory, reaction time, speed and concentration. If a concussion is sustained throughout the season, it MUST be reported to the CCS Athletic Trainer ASAP, in addition to any paperwork that is received from a doctor.
